[W126 Coupe] transmission problem

40K is about 20K overdue. Fluid and filters are Cheap insurance in
relation to what a new transmission costs. I'm assuming, but you did
not mention checking that the level was indeed correct and that you
checked it after @ least 10-15 miles of driving on a level surface?

It could be a low level issue, an internal leak or a front pump issue.
