[W126 Coupe] start-up and gas tank question

Earlier this week I solved my stalling problem on acceleration when I found a leak in a vacuum line. So that is cured. But when I first start the car, it is a really hard start: a bit more cranking and then as it starts it has a hard shutter, as if a motor mounts is going. Once it turns over, all is well. Any ideas on what this might be?
Also, the car has 205k on it. Is there any benefit to actually draining the gas tank to remove any crud that has probably accumulated over the last 20 years?
