 Jeff, The "vacuum thing" allows the lights to be "angled" or adjusted up and down.  Imagine you have low beam and high beam in a normal car.  That adjustment switch gives you a few more settings where you can move them up and down even at the low beam or high beam setting.  Its not the factory option in the US cars, its a euro option that some of us have adapted into our cars as its not that difficult to do.  Like someone mentioned earlier, you can see if it works by parking the car close to a garage at night, and leaving the lights on, move from position to position with that dial and see if the height of the lights changes or not.
