Is it not possible for a lens to go off? A sharp blow enough to dent the filter ring might affect alignment or spacing. I had a Nikkor zoom which failed to focus infinity until "repacked", whatever that means, and it worked well afterwards. Maybe some lenses were taken apart for cleaning and reassembled wrong. PL
